"hello this is (insert your name here), I talked to my bishop for a recommend and I would like to make an appointment to get my patriarchal blessing."

"When would be a good time"
"X Sunday would be good, X time is usually good"

"Hi, bro. _____! I got my blessing recommend from the bishop and would like set up a time to come by and receive my blessing. When is good for you?"

This is his JOB, so he knows what you are calling for.

Just tell the patriarch you want to set up an appointment to get it done. I got mine while I was either a junior or senior in high school which was a couple of years ago.

Call and ask the patriarch if you can set up an appointment to receive your patriarchal blessing. He will take it from there, and schedule the appointment. It will most likely be a few weeks from the time you call, depending on how busy he is and for him to prepare for the blessing as well (see below).

It can be nice to have a family member, or two, or three join you for the experience. It is also a good idea to "prepare" for the blessing by doing some things like prayer, fasting that day to receive of the Spirit, etc.

Good for you!!
