Catholicism was already established, and the urbanity's would naturally be attracted to the establishment.

It was in the cities like Vienna and Hamburg.
People in the country didn't have the ability to hire a protestant preacher and they were mostly living on Church lands.

I may be wrong, but I'm gonna go with D.

In 1555, the Peace of Augsburg gave each German prince the power to choose what religion their territory would be under.
Protestantism spread across Northern Europe because of it.

I understood the idea of protestantism to be a reaction to the catholic religion
which was repressive, and had strict rules that weren't in the bible. I is a more
liberal viewpoint, but the bible teaches we are not to be in contention. That said,
I don't know how else people could have drawn away from the catholic church.
Now, we notice many more 'non-denominational' churches.
